-Patient-specific medication therapy reviews that go well beyond typical drug utilization reviews (DURs)

Origins utilizes only pharmacists specially trained in hospice and geriatric pharmacy.  The medication therapy reviews are performed by a pharmacist rather than a computer program, providing valuable information for optimization of both patient pharmaceutical care and the hospice pharmacy budget. 

The Origins medication therapy review process ensures that clients are meeting and/or exceeding the criteria of the CoPs. Origins MTRs are patient-specific rather than just a computer-generated list of interactions, providing case managers and providers with useful, applicable information for optimal management of each patient.
